Certificate Reprint Request
If your certificate expires within 6 months, you will need to renew your certificate.
We are unable to issue reprints for certificates printed before October 1,2008.
Entry Level Postcard Reprint Request
Course was:
- Completed after July 2000 – The Registry no longer issues duplicate postcards.
A Registry career level certificate shows what positions you qualify for under Department of Children and Families (DCF) regulations. We do have records of all DCF approved entry course attendance taken after July 2000.

- Completed before July 2000 - Contact the agency where you completed the course to request another copy. The Registry does not have Entry Level records prior to July 2000.
- Taken at a college/university - please supply a copy of your official transcript when you submit your Registry application.
